Waiver of Liability & Assumption of Risk
As part of my registration with Player Pathway an online soccer training program powered by Coerver®, I, as parent or guardian, acknowledge that there is inherent risk in the activities involved in the program. I, hereby agree to assume all such risk on behalf of myself, child and family.
Assumption of Risk
I, parent or legal guardian, recognize that severe injuries including bodily injury or death can occur in sports or activities involving height or motion. Being fully aware of these risks, I hereby give consent for my child(ren) to access and use any and all Services of the Website www.coerver.playerpathway.com and access, use and participate in any and all Coerver® Soccer School Services. I recognize that these activities will occur in my home or designated area and that damage could occur to the property either owned by me or third parties or third parties. I assume all risk and liability associated with these activities in my home, personal property, a third party, third party property or chosen area to engage in the Coerver® Soccer School Services. I fully accept all risks associated participation in Services of the Website and/or Coerver® Soccer School Services on behalf of myself, children, and family including respective heirs and successors. I, as parent or legal guardian, am fully and solely responsible for any care or medical attention required by my child(ren) at all times.
Waiver of Liability.
I, as parent or legal guardian, knowingly and voluntarily hereby forever release (and covenant not to sue) Player Pathway, LLC, Sportsmethod USA, Inc, Coerver® brand its respective organizers, affiliates, partners, officers, directors, employees, members, owners, volunteers, agents, corporate sponsors, teachers, guests, and/or representatives and each person individually (collectively the “Organizers”) from any claims or liability resulting from any damage (bodily or property), injuries, death, or any other claims to the fullest extent possible under the law.
Hold Harmless.
I, as parent or legal guardian, further agree to indemnify, and hold harmless Organizers for and from any and all liability for all claims, demands, losses, damages and costs including without limitation reasonable attorneys’ fees, fees that arise out of or in connection with any personal injury to amy child(ren) or others, sickness, accidents, delay, property damage, and/or other loss, damage or expenses of any kind suffered by the my child(ren) in connection with the child(ren)’s participation in the online program.
MY WAIVER AND RENUNCIATION OF CLAIMS IN THIS AGREEMENT EXPRESSLY APPLIES TO ANY BODILY INJURY, DAMAGE, OR ACCIDENT THAT MAY BE SUFFERED BY THE PARTICIPANT OR OTHERS RESULTING FROM THE PARTICIPANT’S PARTICIPATION IN CONNECTION WITH ONLINE PROGRAM.
As the child(ren)’s parent/ guardian I acknowledge that I have read this waiver form and am allowing my child(ren) to participate in Coerver® Soccer School Services.
I, for myself, my spouse, my child(ren) and on behalf of my/our heirs, assigns, personal representatives and next of kin, hereby indemnify and hold harmless all the above Organizers from any and all liabilities incident to my child(ren)’s involvement or participation in this programs, even if arising from their negligence or misconduct, to the fullest extent permitted by law.
I am signing the waiver electronically, and that, under the Electronic Transactions Act, such electronic registration and signature will be valid and enforced in the same manner as a hand-signed document that exists in physical form and that a record or signature may not be denied legal effect or enforceability under law solely because it is in electronic form.
